feat(ui): add cost tracking to usage dashboard and pricing settings
- Add toggle view (Tokens/Costs) to UsageStats component
- Display cost breakdown in usage tables
- Add Total Cost card combined with Total Output Tokens
- Create PricingModal component for editing rates
- Create Pricing Settings page
